Good Morning. My therapist suggested to me the PTSD Workbook by Mary Beth Williams and Soili Poijula.  Simple and effective techniques for Overcoming Traumatic Stress Symptoms.

Anxiety and Phobia Workbook by Bourne.  I got mine from Chapters. It is excellent. My therapist uses stuff out of it.

Texanfemme has the name of a very good one. I can't remember it's name or author as I lent mine out, but I see she still visits . I'll go back a few posts and see if I can find it for you.
