I'm no expert on NRTs I'm afraid, but I do understand the feelings of anxiety attacks. It's not nice at all and I'm sorry to hear that you have to endure these horrid things at the mo.
I went through a patch of dealing with these attacks of anxiety and found distraction the best way to deal with it. You are right to use breathing to relax you physically, but then I would usually try to force my thoughts elsewhere, to reduce the 'panic'. One thing - NOTHING ever happens during these panic attacks. Nothing bad will happen to you.
Maybe you should have a word with your doctor, to check on the meds and the patch.
