I am sort of doing 3 and 4 together, but I remember being interested in how our behavior can change our thoughts rather than waiting for our thoughts to change first before we start behaving differently. For me, this means that even if I don't feel like doing something (like going grocery shopping), I will feel better if I actually go do it rather than sitting at home. Before I was on meds though, I could have not accomplished this. Bipolar depression didn't/doesn't seem to respond to CBT too well without mood stabilizers, but it's working out great now that I am stabilized on meds. 
 
I remember trying to do things (behaviors) that should have been enjoyable, but just being miserable the whole time no matter what activity I did. Now, even if I don't feel like doing anything, if I push myself to participate in an activity, I usually start feeling a bit better. So I can definitely see how CBT helps.
 
I don't remember the: " How to design a treatment plan" part. I will have to go back and read it again.
