Do you have a therapist? If so, maybe he or she can put you in touch with any support groups in your area. I'm sorry you're faced with such a challenge at such a young age but look at it this way, if you can get things figured out now you've got many, many, many good years ahead of you. You can also try your local community mental health center for possible groups. Good luck!
