Step by step not always convenient to life

Thanks Vickers4 for your great response to my fear of going out and going out in the car. The only thing is, I know this programme is designed for us to face our fears step by step by life doesn't always allow us this pleasure! How can I overcome a fear of going in the car (taking it step by step as in sitting in it first, going round the block next, then a little further) when my boyfriend isn't available to do this step by step plan as I don't drive, so therefore, he expects me to go further at a weekend not just round the block when he is available! Plus, if I have an appointment which takes me out of the programme's step by step idea and into high levels of anxiety (no build up at all) what can I do? I think facing fears gradually is good and allowing the sensations to subside but life has a habit of saying; 'Nope, no time for this gradual exposure, you HAVE to go to ... today which is x many miles away' What in the world do you do then?? Please help.
